All bariatric surgeries are required to be completed under your SurgeryPlus benefit with a surgeon in the SurgeryPlus network. Bariatric surgery is not covered under your Aetna or Highmark Delaware non-Medicare health plan and will not be eligible for a financial incentive.

  • Custom Care Management Unit (CCMU): Call 1-844-459-6452 to be connected to specially trained wellness professionals. Your dedicated Registered Nurse Case Managers are considered to be a “Nurse in the Family” that can assist you with various men’s* health needs such as:
    • Planning for care at the hospital or understanding discharge instructions.
    • Understanding prevention and preventive screenings (i.e., colorectal exams, annual physical exams, etc.).
    • Review and understand your doctor’s instructions and/or treatment options.
    • Keep up with and plan for doctor’s and/or specialist’s appointments.

    There are Behavioral Health specialists available for support with mental and behavioral health concerns, treatment options, and locating an in-network provider (in-person or virtual visits).

    There are Wellness Coaches available for goal setting and accountability related to weight management, stress management, tobacco cessation, sleep improvement, exercise planning, and healthy eating.

    There are Social Workers available to connect you with local resources such as meal delivery and food supplies, transportation assistance, and other helpful support and resources.

  • The Highmark Community Resource tool can be accessed to locate specific services in your area should you need them.

  • Stay on top of your health by keeping up with your Preventive Screenings.
