If you are a newly hired benefit-eligible employee, please review this Welcome Letter  for important information about your benefits. Then, complete the following:

1. Use the links below under “I WANT TO...” “INFORMATION,” and “ENROLLMENT TOOLS” to review the benefit options and resources available to you.
2. Complete and submit the Health, Dental, and Vision Enrollment/Change Form  along with any supporting documentation to your organization’s Human Resource/Benefits Office within 30 days of the first of the month that benefits coverage becomes effective.
   • Visit the Spouse and Dependents page for information about enrolling a spouse and/or dependent children in your benefits.
3. To enroll in Life Insurance, Accident and Critical Illness Insurance, Flexible Spending Account, and/or Pre-Tax Commuter Benefit, follow the instructions on the New Hire Checklist .

If you want to make a benefit or dependent change as a result of a qualifying event during the year, you must contact your organization’s Human Resource/Benefits Office within 30 days of the qualifying event and request the change.
