Highmark Delaware – Diabetes Resources


The following resources are available to employees, non-Medicare pensioners and their covered spouses and dependent children who are enrolled in a Highmark Delaware plan.

Your CCMU Health Coach can help you work more closely with your doctor and get more involved in taking good care of yourself. Health Coaches are specially-trained registered nurses, therapists and other medical professionals. Get the support you need for managing your diabetes, losing weight, eating better, quitting smoking and managing stress. Call 1-844-459-6452.

The Diabetes Prevention Program (DPP) is available to members who meet program eligibility criteria. The DPP is evidence-based, recognized by the CDC and can help participants eat healthier, increase physical activity, lose weight, overcome stress, boost energy and reduce risk of chronic conditions including type 2 diabetes. The Highmark Delaware Member website includes an extensive library of articles, recipes and videos created to improve members’ 'health smarts' and inspire members to live happier, healthier lives.

Diabetic education provides instruction on the care and treatment of diabetes, including foot care, eye exams for diabetic retinopathy, blood sugar monitoring, medication management and diabetic nutritional counseling. Diabetic education can be performed by either physicians or Certified Diabetic Educators, either on an individual basis or in a group setting.

Services are provided for the assessment and guidance of members at nutritional risk due to nutritional history, current dietary intake, medication use or chronic illness. Nutritional counseling is indicated for certain diagnoses, including diabetes.
