Enrollment Outside of Open Enrollment - State Agency Employees (Including DOE)

New Hire

If you are a newly hired benefit-eligible employee, please review this Welcome Letter for important information about your benefits. Then, complete the following:

  1. Use the links below under “I WANT TO...” “INFORMATION,” and “ENROLLMENT TOOLS” to review the benefit options and resources available to you.
  2. Complete and submit the * Health, Dental, and Vision Enrollment/Change Form along with any supporting documentation to your organization’s Human Resource/Benefits Office within 30 days of your date of hire.
    • Visit the Spouse and Dependents page for information about enrolling a spouse and/or dependent children in your benefits.
  3. To enroll in Life Insurance, Accident and Critical Illness Insurance, Flexible Spending Account, and/or Pre-Tax Commuter Benefit, follow the instructions on the New Hire Checklist .

Qualifying Event Status Change

If you want to make a benefit or dependent change as a result of a qualifying event during the year, you must contact your organization’s Human Resource/Benefits Office within 30 days of the qualifying event and request the change.
